ASSUMPTION OF RISK, WAIVER AND RELEASE OF ALL CLAIMS AGREEMENT — Kinzler Foundation Team, 2026 Grand Rapids River Bank Run · 5K Walk Participation Please read this agreement carefully before selecting "YES, I AGREE" below. This agreement covers your participation in activities organized by the Jason Kinzler Family First Foundation (the "Foundation") in connection with the Foundation's team at the 2026 Grand Rapids River Bank Run, including the team meet-up, the 5K Walk as a member of the Foundation team, the team photo, and the post-walk gathering (collectively, the "Team Activities"). This agreement is in addition to, and does not replace, the official Amway River Bank Run waiver you must separately sign as part of your official race registration. You must complete both. By selecting "YES, I AGREE" below, I confirm that I am 18 years of age or older, or that I am the parent or legal guardian of the participant if registering someone under 18. I am physically fit to participate, have no medical condition that would make participation unsafe, and have consulted a physician if I had any doubt about my fitness to participate. For myself, and on behalf of anyone else I have registered, my (and their) heirs, executors, administrators, trustees, and assigns, I agree as follows: (A) ASSUMPTION OF RISK. I understand that participating in the Team Activities, including walking the 5K, may be hazardous and carries inherent risks of accident, injury, disability, loss, and/or damage regardless of severity, including but not limited to death. Such risks include but are not limited to: tripping, slipping, or falling on uneven surfaces; collisions with other walkers, runners, vehicles, bicycles, strollers, or animals; weather-related conditions including heat, cold, rain, or storms; cardiovascular or other medical events; and injuries arising from crowd density.
