JUNE SIMMONS - 2026 Dignity Awards Gala Community Champion

JUNE SIMMONS is the Founding President and CEO of Partners in Care Foundation, a nationally recognized nonprofit in the San Fernando Valley dedicated to improving health outcomes by uniting healthcare and community-based services. A respected leader in whole-person care, June has spent her career transforming how care is delivered to older adults, individuals with disabilities, and people with complex health and social needs. She led the creation of the nation's first Community Care Hub — a model that changed how healthcare and community-based organizations work together to address the non-medical drivers of health outcomes. Today, that model has been replicated nationally, and Partners is recognized as one of the nation's leading Advanced Community Care Hubs.

Under June's leadership, Partners has grown into one of California's largest providers of home- and community-based services and a national leader in care transitions, medication safety, and community-integrated care. She has also shaped state and national policy through numerous advisory roles, including as National Co-Chair of the Partnership to Align Community Care, where she works with leaders across the country to advance sustainable models connecting healthcare and community-based services. Her commitment to innovation, collaboration, and service continues to improve lives in communities nationwide.
