KC Kansas Angels Golf Classic Waiver

In exchange for my being allowed to participate in KC Kansas Angels Golf Classic Event (herein activity), I, (being at least 18 years old), or my parent/guardian, agree to be bound by each of the following:

Assumption of Risk - I assume all risks, known and unknown, in any way connected with my participation in the Activity.

Waiver and Release - I waive and release National Angels Parties, the Chapter (KC Kansas Angels) identified above, any affiliated organizations of either of the foregoing and their respective directors, officers, employees, agents and volunteers (collectively, the National Angels Parties), from all claims that I may have for any liability, injury, loss, or damage in any way connected with my participation in the Activity, including but not limited to any injury or loss caused in whole or in part by the negligence or misconduct of any of the National Angels Parties.

Photo &  Media Release - I hereby grant KC Kansas Angels and National Angels and its Affiliated Chapters, and their agents the absolute right and permission to use photographic portraits, pictures, digital images, videos and content with the names* of myself or my golf team*, or in which myself or my golf team* may be included in whole or part, or reproductions thereof in color or otherwise for any lawful purpose whatsoever, including but not limited to use in any publications and media by KC Kansas Angels, National Angels and its Affiliate Chapters or on National Angels and its Affiliate Chapters websites/social media pages without payment or any other consideration. 

I hereby waive any right that I may have to inspect and/or approve the finished product or the copy that may be used in connection therewith, wherein myself or my golf team* likeness or names* appears, or the use to which it may be applied. This authorization extends to all languages, media, formats and markets now known or hereafter devised. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing.

I hereby release, discharge, and agree to indemnify and hold harmless KC Kansas Angels, National Angels and its Affiliated Chapters, or their agents from all claims, demands, and causes of action that I or my golf team* have or may have by reason of this authorization for use of my golf team*, photographic portraits, pictures, digital images or videotapes, including any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said images or videotapes, or in processing tending towards the completion of t
